I made Iocane Powder from The Princess Bride in Minecraft with command blocks.
You just need to throw a swamp flower, poisonous potato, and spider eye on the ground, then light it on fire to get the powder. Then it can be mixed with any potion without visibly altering it in any way.
All potions will still function completely as normal -- providing their usual effect as expected. However, after about twenty seconds (though I shortened the time for this gif), the consumer will instantly die.
This does not work with splash potions, because that would just be brutal.
My next goal is to allow the player to consume smaller amounts of the powder to become less susceptible to the effects...

It's odorless and dissolves instantly in liquid.
However, I wasn't entirely able to eliminate the slight sweet taste...

It does have a twenty-second delay before you actually die. Perhaps you didn't wait long enough?
Other than that, it does rarely kill the powder without actually poisoning the potion. You could have just been unlucky.
