Issue Renaming on Anvils

For the past few days, I haven't been able to rename items on an anvil. When I place a name tag, pick axe, fishing rod, etc in the first anvil slot and enter a new name, a new item doesn't appear in the last slot to remove. I've tried on old and brand new anvils with the same issue.
