Glitching on completive parkour map 'Venice'

When you jump from the last lily pad to the oak logs or when you jump from the starting point to the nails and than the hammer it glitches out and you get sent to your last checkpoint. There might be more glitches beyond the two I listed but this is just the farthest I've gotten.
