Horse and anything on horse disappears

I've lost a total of 4 horses in our realm for some reason. I would log out, log back in, and the horse I've ridden is gone. It can be on a boat, I'll be riding it, or leashed somewhere, and it'll disappear along with the items it touches (including boats, leashes, etc.).
