Broken Nether portals.

Nether portals are not working properly. All nether portals made in the over-world transport you to one nether portal that was made at the start of our world. Even after destroying all portals, a new one transported you to the same portal in the nether. This issue is very, very annoying because it is impacting massive game play on the realm.
