Chat filter censoring or corrupting randomly

The vulgarity filter censors (or maybe corrupt) randomly some non-vulgar text. Some words can't be written in a certain sentence but can in another one and some words can't be written at all. It takes place in books, chat and signs at least. It censors with "########" but I saw that real bad words are censored with "******". In books, it removes all the line breaks.
