As soon as I enter Nether Portal death message “tried to swim in lava” appears

As soon as I enter a nether portal on my Minecraft Realms server, I immediately appear as trying to swim in lava. Checked the portal on a copy to a single player world and the portal seemed to be clear and a safe spawn, and didn't die.
