Hostile Mobs Spawning in a Lit Area

There's hostile mobs spawning in areas where there is torches, lanterns, etc. they keep spawning in slightly shaded areas or completely lit areas. We've tried to totally cover a floor with torches and lanterns, to no avail. Hostile mobs spawn, no matter what we do.
