Nether portal won't teleport me

When stepping into a nether portal I am not teleported.  The teleport animation does not play.  I have tried destroying and rebuilding the portal, I have tried logging out and logging back in, and I have tried moving the portal and nothing works.  I've only managed to successfully teleport by standing in the portal and then logging out and back in.
