i have mobs and disappearing probs!

i have mobs spawning on chests. in torch and glowstone spammed places. i have 2 villagers and minecarts disappearing when i go through the nether portal. i have blocks placing below blocks at random times. the biggest one is my recipe's are nowhere to be found. i have been using wiki for crafting since update! (Great update by the way sincerely) please someone help please!?!
