Wheat appearing when planting seeds

When planting seeds, I have discovered that sometimes as soon as I plant a seed it turns into fully harvestable wheat. Hope this can be resolved, please watch the video attached.

Update: I have also now realised that it does this with potatoes, not sure if any more crops are affected.
