Invisible Blocks when having high levels of Efficiency.

I don't have a picture for this, but whenever I Mine blocks in general sometimes the blocks don't break, and become not visible to me. Thus I take damage as if i'm suffocating within something. not sure if anyone else is having this issue but it's been happening to me allot.
