Disappearing blocks

I was building the nether reactor and i misplaced a block so i tried to destroy it and got nothing back also it happens randomly

What I expected to happen was...:
Get the block back
 
What actually happened was...:
No blocks at all
 
Steps to Reproduce:
1. Place a block destroy it(it works randomly)
