I died when I was out of the game

I was playing in my survival world and I I was outside at night then I pressed save & quit. When I came back I was dead but luckily my items didn’t disappear but I lost my totem of undying (because I was holding it in my offhand). I think a mob might have come and killed me when I was out of the game but HOW!
