Instances of damage have a small chance of giving the player several hearts of permanent absorption.

When taking damage, the player will, on occasion, acquire several hearts of permanent absorption. In one case, I was quite low on health, and the base hearts wobbled as if a totem of undying had activated, but the absorption did not dissipate. This appears to have a chance of proccing only when the damage would be lethal if not for the absorption.
