Game repeatedly disconnects while playing cross-platform.

I can play multiplayer for a solid 5 minutes before the game disconnects me with a message that reads "disconnected from server". When i'm taken back to the main menu, the words disappear and I either have to wait 5 minutes on the menu, or restart the game completely. These issues are shown here;

this has been extremely irritating for me and I am unsure if I am the only one with this problem.
