Items vanishing while entering a nether portal and dying.

If you somehow die right before you get teleported to the nether dimension, most of your items may disappear with no way to reclaim them in Survival. 

 

Easy way to repeat this bug is to /kill or kill yourself with instant damage splash potions right as your about to poad into the nether.
