Killed instantly by non existent zombie, all carried items disappeared immediately.

While swimming in a river my health, which was almost at full, dropped to zero within approximately 2 seconds. I died respawned and sent a friend who was nearby to the location of my death. He arrived within minutes but all of the items I had on my were gone. In my experience the items should still have been there. He also saw no evidence of the apparently extremely effective zombie.
