Not able to engage in melee combat

After killing a skeleton with my bow, I turned on a spider switching to my iron sword and was not able to swing the blade. Shortly after dealing with the spider, i discovered I couldn't swing my right hand. I could open doors and activate items, but could not attack except for a bow and arrows. I've tried shutting down the ps4 and even switching controllers with no success.
