Item Duplication Glitch

Duplicate items in block containers, (Chest, Barrel, Hopper etc.)

This occurs when the container block is changed position (example: that a piston moves the block), just when an item is extracted from the container block, the item is successfully extracted, but in turn also remains in the container. Causing the item to be duplicated.
