Crafting and trading causes items to vanish?

Whenever I attempt to craft an item or trade with a villager, the block or item I am using gets consumed but I do not receive anything in return. Bones disappear but no bone meal shows in inventory, fish vanishes when trading and no emeralds are received in return.
