Hotbar highlighter highlighting incorrect equipped item

Sometimes if I happen to hit the shoulder button on my controller just right, the hotbar highlighter moves, but the item in my hand doesn't change.

Steps to reproduce:

1.) Equip an item in your hotbar

2.) Hit the trigger and the right bumper just right

3.) The item equipped is not the item highlighted.
