Progress disappearing upon changing dimensions or leaving the world

Upon entering the Nether changes to the world (Blocks placed and broken, entities) are reset to how they were previously. Whenever I leave the world everything (blocks place and broken, enderchest, inventory, entities) is reset to how it was. So far nothing has reset to how it was longer than a day before.
