Game Freezing and/or crashing shortly after going to the end

As the title says sometime over the past few weeks my game has started freezing and/or crashing just after going to the end and walking 10-30 blocks. When I say freeze, it isn't the quick freeze that happens when the game autosaves, this is a full system lockup for anywhere from 2-5 minutes. Most of the time I have to force close the game.

This affects both the Windows 10 and Xbox One versions.
I have a realm that I have been paying for for a few months now.

Steps I have taken:
Removed resource packs.
Lowered rendering and particle distance.
Reuploaded the world to the Realm.
Tried offline.
Tried creative.

Sometimes it will unfreeze after a few minutes and then I walk another 20 blocks and then it will freeze again.
