Keeps crashing after playing for a few seconds and not loading the terrain properly

Every time I load an world doesn't matter if it's old or new it crashes after a few seconds. Usually it crashes while breaking a block. My recent observations is also when I create and load a world it has the terrain glitched out and don't see any blocks.
