Duplicated Achievements in in-game menu

Achievements sometimes gets duplicated, says I've completed 'Passing the Time' twice or three times. The gamerscore count goes to 3000 or 6000 and claims that there are 141 or 316 achievements instead of the usual 2070 gamerscore and 91 achievements.
