graphical problems and crashes

every about 5 minutes i get a few lines that go across my screen when running or turning to quickly, also im getting game crashes sometimes when the save icon appears. Once it totally froze my console up and had to hard reboot. It crashed a total of 5 times now in a 48 hour period now
