Sometimes when i dig or place blocks they disappear and i die from falling

 I'm playing Windows 10 edition with friends on the Xbox one edition and I don't know if they run into the same problem but check that version as well. Anyways, on occasion, when I place a block it disappears and is placed into my inventory, or when I break some blocks and then a bunch disappear, which I don't mind, however, when you have quite a bit of valuables in your inventory and you fall into lava or die in the void, its pretty angering. Please look into the issue, and Thank you.
