Hostile mobs disappearing and reappearing in hitting range

Hostile mobs will disappear while they are headed towards me, and then reappear just as they get in range to hit me. This caused me to die 4 times in the nether with all mobs disappearing just as I shot at them, and then showing back up to hit me. It also happened with Ghast shots that I didn't see coming until just before they hit me.
