Chorus Fruit Sound + Replaceitem Command Offhand Bug

1-There Is No Sound When Chorus Fruit Teleports You.
2-When You Use The (/replaceitem) Command In A Command Block, For Example:
/replaceitem @p slot.weapon.offhand 0 totem
So It Is Supposed To Switch/Give The Closest Player A Totem Of Undying In Their Offhand, However It For Some Reason Gives The Totem To All Of The Players In The World, I Haven’t Really Tested It With Other Items Rather Than The Totem Like Arrows And Maps, But Yeah.
