Items are missing from the creative inventory

Random items from the block menu, whether in creative or survival, go missing. Typically, a reboot or relog temporarily fixes the issue and the blocks/items are put back in the proper place. I have experienced this issue on Xbox One and iOS, but most frequently on the former. I've not noted any pattern to when items/blocks go missing.
