Some items disappear from my inventory bar and throws itself for no reason.

This happens sometimes when I am in a realms that something disappears from my inventory bar, especially my diamond picks. I lost 3-4 diamond picks in less than 3 weeks and it's not because they broke. ^^

Also, some blocks I hold in my hand throw themselves for no reason at times! :/
