/replaceitem bug

It is weird bug. When you use this command (screenshot 1) block will be fill your helmet slot. There is two bugs
1: Pumkin UI shows but there isn't Pumkin (Screenshot 2)
2:Block doesn't shows on your player (only pumkin)
