minecraft world wont reload

so i am into making mods and i update the one a made frequently, so it was easy for me to notice that when i try to load the new, updated version of the mod into any world i made that has the OLD version of the mod, it won't reload and add the new stuff, even if i do "/reload all". can someone please help me fix this??
