Hearts don't fully recover

Even when you're full, your hearts will not fully recover, and you will stop at half a heart missing.

If you take off all your equipment and die, you will fully recover, but if you take damage after re-equipping it and recover, you will not be fully recovered and will return to being half a heart missing.
