Cursor doesn't lock to game

Cursor doesn't lock to game, when you look to the right the cursor follows to second monitor.  Without a second monitor you still find problems when you look up or down because an interface opens and if you click, it clicks whatever you are hovering over. (pictures taken at the same time)
