armor drops after any damage

When wearing diamond armor with Mending and Unbreaking, upon any damage either by mob or fall, all armor falls off and needs to be re-equipped. 

I would expect armor to take damage, but not fall off. 

“Keep inventory” is turned on. 

“Minecraft Essentials” add-on is applied. 

“headstone” is applied.
