Portals dont seem to link up 90% of the time and just makes a new one in a random location

Me and my friend were playing and he left the nether because he had gotten what he had needed and instead of spawning him at his base it spawned him around 100 blocks away and under the ocean. By the time he got back to his stuff it had despawned
