Items disappearing from inventory due to logging off too quickly

Whenever I log off the game and log back on later it basically forgets anything I’ve done about a minute before logging off as if it never happened so if i slept for example and then logged off i would log back in and be standing next to the bed in the middle of the night still. and more recently having the issue of moving stuff from inventory to a chest or from a chest to inventory and logging off and logging in and the item disappearing completely, being neither in the chest or inventory
