Structure mobs don't spawn or respawn

The nether fortress is only spawning the puglins and the blaze spawner works but there's no wither skeletons or blazes spawing at all. Same with guardians often they don't spawn correctly and once you killed a captain at a outpost they're not spawning anymore please fix this big issue.
