Teleporting when using lever

I went to close my hidden room and upon hitting the lever I got teleported 150 blocks away in the X axis. I was able to recreate it more than once and luckily got a video of it (I have a longer version but I had to shorten it to include it here). It seems to only be that lever so idk if it’s a chunk issue or possibly a piston issue. My world is originally from the Xbox 360 and got transferred to bedrock on the Xbox One so it has a lot of oddball bugs.
