Can't break blocks or punch

Playing on my friends realm and everything was fine but then I wasn't able to punch or break blocks.  I don't know how to reproduce it since i still can't punch.  Even in single player and on another server i still cant do anything with left clicking.  Dont know full versions it happens on but this is my experience.
