Bugging blocks and portals

I’ve created 2 kinda of good survival worlds and both of them ending up having some type of glitch then first one I made a neither portal and broke it to try to get a new see if I could get a new location and it’s been spawning random neither portal around my survival world and I can give pictures and clips of it 

 

2nd issue

every time I place a block and break it it’s vanishes for no reason when I was break a block of dirt with a shovel a stone shovel it broke and didn’t make a block and when I try to break wood with a stone ace it dosent turn into a block

 

broke it 3 times btw
