Dying from wither effect while going through end exit deletes items

I was playing on an smp (realms, but not sure that was what caused it so tagged as Bedrock), was fighting a wither in the end, and went through the exit portal on half a heart while having the wither effect (holding a totem). It showed the death message died in the void, and I lost all items (checked world spawn, my bed, exit portal). Haven't been able to reproduce bug.
