"ULTIMATE CUSTOM HEARTS" mod doesn't work

I have bought the mod "ULTIMATE CUSTOM HEARTS" by Shapescape - https://www.minecraft.net/en-us/marketplace/pdp?id=a0fdbc28-3b61-448b-9269-cd8f153a6621 - and all it does is create a blank world where you can't move, and then freezes my game.
