Newly placed blocks disappear, broken blocks reappear + other issues

When I place new blocks, they disappear within a few seconds. When I break blocks, they reappear within a few seconds. The TNT takes a long time to explode. If I hit a mob, it doesn't hurt them, they fly into the air for a short period then come back down (they only take damage after they fall). Cannot pick up items.
