Cannot Chat With Other Players

Occurs on all devices - Google Nexus 5, iPad Mini 4, Windows 10.

Occasionally when I attempt to chat I get the message "You cannot chat with other players because of how your Xbox Live account is set up. This can be changed in your privacy & online safety settings on Xbox.com".
All settings are enabled on Xbox.com and restarting the client generally allows me to chat.
