Tools renaming themselves to ###########

I had my bow in my enderchest when I logged off of a realm I'm in. When I logged back on, it had renamed itself to ########### . Today, which is 3 days after that, my axe did the same thing. Logging back on to take pictures of these tools revealed that my pickaxe also did the same thing.
