Censoring issue and Nether portal

So when I try to name something or add an enchantment book to, let's say a crossbow, it will censor out the crossbow part despite the fact the name was never changed. I have not tried it on the signs, but the chat seems to be down, too; messages aren't going through typically unless you use the /w. As for the Nether portal, no matter what we do when we try to return to the overworld, it will send us to a cave in the middle of nowhere. We've attempted to break every portal and retry; this includes the ones in the overworld and the Nether.
