Jumping through End Portal Crashes Game

I haven't managed to take a screenshot of this bug yet but there have been a handful of instances where when I jump through the end portal, the game crashes without even loading the end. We typically play w 2-3 people on the server at a time. 

 

One of the instances was after a death. The other instances I don't remember anything specific taking place.
